pkg/aflow: handle common LLM mis-behaviors wrt tool calling
Gracefully handle (reply to LLM with error):
- incorrect tool name
- incorrect tool arg type
- missing tool arg
Silently handle:
- more than one call to set-results
- excessive tool args
Fixes #6604
